Form UB 92 is also known as a Uniform or Universal Billing form. It is used in the healthcare industry to submit insurance claims to Medicare or other health insurance companies. Completion of this form helps insurance companies decide whether the healthcare provider should receive reimbursement. Once this form is completed, a copy should be sent to both the patient and the health insurance company. The healthcare facility should also maintain a copy for their records.
