A VA Form 21-674 is known as a Request for Approval of School Attendance. This form is used by the United States Department of Veterans Affairs. This form will be used in order to request that a veteran’s child between the ages of 18 and 23 continue receiving veterans benefits based on their enrollment in school. This form can only be completed after a child reaches the age of maturity, which is typically age 18.
This form requires information about the claimant and the student. Personal identifying information about both parties will be needed, including full name and social security number. The information for the student’s school should also be included. Detailed information about the student will be needed, including their current income and expenses, major in school, and their current enrollment hours. This information is used to determine the eligibility of the student to continue receiving VA benefits.
This form is used by someone who is receiving or claiming benefits for a veteran’s child who is at least 18 but under 23 and attending school.  The veteran’s child should complete the form only if he or she has reached the age of majority in his or her state and is or will be entitled to receive direct payment of VA benefits.
