The VA Fiduciary’s Account or Federal Fiduciary’s Account is a document formally known as VA Form 21P-4706b. This form is used to detail the receipt and use of funds being managed for a beneficiary during a specific period of time. VA Form 21P-4706b requires pieces of information about the fiduciary, the beneficiary, and the veteran. All the money received and spent will need to be accounted for, as well as an accounting of all savings bonds and other assets owned by the estate.

Your obligation to respond is required to obtain or retain veterans benefits. The information will be used to ensure the proper administration of the beneficiary's income and estate. Failure to furnish the requested information may result in the suspension of payments and/or the appointment of a successor fiduciary.
