What is a social security card template?

Your social security number (SSN) is a nine-digit number that identifies you with the Social Security Administration.  This number is used to identify you to employers, record your earnings, and to collect Social Security or other government benefits.

If you need to obtain a Social Security card, you should only do so through the Social Security Administration.  You should not use the social security card templates that you find online. These are not legally valid.

Common Uses

You need a SSN to:

  • Get a job
  • Collect Social Security benefits
  • Access government benefits
  • File taxes with the IRS

How to complete a social security card application

To apply for a Social Security card, you need to provide at least two forms of identification that prove your age, identity, and U.S. citizenship or legal work status.  Non-U.S. citizens who do not have DHS work authorization must prove that they have a valid reason for requesting the card.

Documents that prove age include:

  • U.S. hospital record of birth
  • Religious record showing your age or date of birth that was created before age five
  • Passport
  • Final adoption decree

Documents that prove identity:

  • U.S. driver license (you cannot use a foreign drivers licenses)
  • U.S. State-issued non-driver identity card
  • U.S. passport

If you do not have one of these documents, the Social Security Administration will ask to see other documents including:

  • Employee identification card
  • School identification card
  • Health insurance card (not a Medicare card)
  • U.S. military identification card

All identifying documents must be current and show name, birthdate or age information, and preferably a recent photograph.

U.S. Citizenship should be proved through U.S. birth certificate, U.S. Passport, Consular Report of Birth, Certificate of Citizenship, or Certificate of Naturalization.  U.S. Immigration Status should be proven through an unexpired document from the Department of Homeland Security, such as Form I-551, I-94, or I-766.

The application for a Social Security Card requires you to provide:

  1. Full name (First, Middle, Last)
  2. Full name at birth
  3. Any other names used
  4. Any other Social Security Number (SSN) previously assigned
  5. Place of birth
  6. Citizenship
  7. Ethnicity
  8. Race
  9. Sex
  10. Parent/Mother’s name at birth
  11. Mother’s SSN
  12. Father’s name
  13. Father’s SSN
  14. Whether you have ever applied for a SSN before
  15. Name on most recent Social Security Card
  16. Any different date of birth used on earlier application
  17. Current date
  18. Daytime phone number
  19. Mailing address
  20. Signature
  21. Relationship to party that card is being requested for

Social Security Card Templates

It important to know that you should not use the social security card template downloads that you find online.  Many website templates allow users to create fake social security cards. These sites typically have a blank social security card template that you can fill in yourself.  These sites sometimes say that the free templates should only be used to create a "novelty social security card."

These sites often provide you with a psd template or a social security card template pdf.  The psd template is something that you can modify if you have a program like Adobe. Pdf documents are more difficult to modify.

If you choose to use these editable social security card templates, know that you could be committing a crime.

If you use these blank American social security cards, some of the potential penalties include:

  • Using false documents to be employed: A maximum penalty of 10 years without parole in federal prison and a fine up to $250,000.
  • Making a false statement on an I-9: A maximum penalty of 5 years in federal prison without parole and a fine up to $250,000.
  • Misusing a SSN: A maximum penalty of 5 years in federal prison without parole and a fine up to $250,000.
  • Making a false claim of resident alien status: A maximum penalty of 5 years in federal prison without parole and a fine up to $250,000.
  • Using false documents with intent to defraud the United States: A maximum penalty of 15 years in federal prison without parole and a fine up to $250,000.
  • Possession of false United States documents: A maximum penalty of 15 years in federal prison without parole and a fine up to $250,000.
  • Aggravated identity theft: A mandatory penalty of 2 years in federal prison to run consecutively to any other sentence, and a fine up to $250,000.
