A WH 347 form is used by the United States Department of Labor. It is specifically related to the wages and hour division (WHD). The form is used for payroll purposes, and it is typically filled out for contractors who want to submit certified weekly hours worked and amount owed.
The form has space for multiple employees under the contractor. Each individual's name and identifying number should be listed. Then, each employee's line can include what times and dates they worked, what their rate of pay is, and what the gross weekly payment should be. There are also boxes for deducting the appropriate taxes and other deductions.
Before submitted this weekly payroll form, a designated representative for the contractor must sign and date the WH 347 form. The information on the certified form will be checked before payments are made to the employees.
Form WH-347 is commonly used by contractors and subcontractors to submit their weekly payrolls as required by their federal and federally-aided construction contracts.
This form is mandatory for covered contractors and subcontractors performing work on Federally financed or assisted construction contracts to respond to the information collection contained in 29 C.F.R. §§ 3.3, 5.5(a). The Copeland Act (40 U.S.C. § 3145) contractors and subcontractors performing work on Federally financed or assisted construction contracts to "furnish weekly a statement with respect to the wages paid each employee during the preceding week."
U.S. Department of Labor (DOL) regulations at 29 C.F.R. § 5.5(a)(3)(ii) require contractors to submit weekly a copy of all payrolls to the Federal agency contracting for or financing the construction project, accompanied by a signed "Statement of Compliance" of certified payroll, indicating that they are correct and complete and that each laborer or mechanic has been paid not less than the proper Davis-Bacon prevailing wage rate. The DOL and federal contracting agencies review this information to determine that employees have received legally required wages and fringe benefits for all contracts subject to these rules.
