This form will be used by the Department of the Army in the United States. Known as a Change of Address and Directory Card, this form is used for helping Army soldiers get their mail delivered to the correct address. Filling out this form is voluntary but if an Army soldier or civilian does not use this form to correct their address, they may not get their mail.
