A Form G-325A is used by the United States Citizens and Immigration Services department. This document is known as a Biographic Information form, and it will be reported to the Department of Homeland Security in order to have a record of information about a prospective U.S. citizen or immigrant.
This one-page form will require a lot of information about the applicant. This includes their full name, current country of citizenship, and the country of birth. Former names should also be included so a full background check can be conducted. The applicant will also need to include this information for their family, including their current or former spouse.
Residence information for the last five years should be listed to ensure the department can get any relevant information about you. Employment information for these five years will also be needed. This information will help determine the purpose for the application and can affect whether or not it is approved.
A Form G-325A is commonly filled out by anyone who is applying for an immigrant visa or green card. There are a few exceptions. If you are under age 14 or over age 79, then you are not required to fill out Form G-325A.

Recently, the USCIS consolidated Form G-325A with Form I-485 (Application to Register Permanent Residence or Adjust Status), so you no longer have to file G-325A during the adjustment of status process.
