    • Whether owners, officers, directors, key employees, or supervisors have ever been employed by or performed similar work for any of the alleged, acknowledged or possible affiliates
    • Whether at the time or bid opening or application for assistance or at present any services have been performed by applicate by any of the alleged, acknowledged or possible affiliates, or vice versa.
    • Whether applicant shares facilities, equipment, or personnel with any affiliates
    • Whether applicant was assisted in preparing the bid or application by any affiliates
    • Whether there is or was any financial obligations between applicant and affiliates
    • Whether there are any individuals who sign documents for applicant who receive indemnifications who are not owners, officers, directors, employees, partners, or principal stockholders of applicant
    • Whether any family member of an owner, partner, officer, director, or principal stockholders has an ownership interest in any affiliate
    • Whether SBA has ever determined applicant to be affiliated with any affiliates
    • Whether any of the affiliates will provide an indemnity or guaranty to a surety to facilitate a contract award
    • Whether any affiliates arranged for any subcontractors required for performance of the contract or any contract awarded in the past two years
    • Whether there have been any actual or proposed subcontracts between applicant and any affiliates
    • Whether any affiliates will perform more than 25 percent of this contract 
    • Whether the termination of applicant on a contract would cause financial impact on any affiliate
    • Whether the procuring activity mailed your current bidding documents to any address other than the applicant’s current business address
