A DS-3053 form is used by the U.S. Department of State. It is used when filling out a United States passport application (Form DS-11 “Application for a U.S. Passport”) for a minor child under the age of 16.  This form must be filled out by an adult who is the parent/guardian of the minor child. The DS 3053 form is only used if the parent or guardian cannot present the completed application for the child in person at an appropriate passport agency, acceptance facility, or U.S. Consulate if abroad.
Because this form is used instead of in-person application, it must be signed in front of a notary public and notarized in order to be legal. Written consent must be provided, and the consent will become invalid after 3 months. A clear photocopy of the front and back of the non-applying parents' government-issued photo identification must be presented to the notary with the written consent. Â This means you should submit the completed application to the issuing agency as soon as possible. Be sure to note any special circumstances before finalizing the application.
